IAF Innovations!
IAF’s indigenous Patient Transfer Units (PTU's) developed by Gp Capt M C Joshi, a critical care specialist and anesthesiologist of the Indian Air Force (IAF) based at Jorhat.
The PTU is basically a 'stretcher-trolley' combination where 'patient-management' can be sustained for eight hours at a stretch; tethered to various life-saving equipment during transit, thus making most of the 'golden period', the vital hour that is critical to stabilize and prepare a patient before specialist intervention, even while transit in air.
Earlier we used to import from the US. A similar off-the-shelf 'Life Support Trauma and Transfer Trolley' (LSTATT) would cost nearly $180,000 in the US way back in 2007. The cost of the IAF PTU is about $30,000. This can turn into a boon for a country like India where cost is a major factor.
The PTU has been put to use even when the IAF has been involved in relief operations, be it in Sikkim after the earthquake in 2011, Uttarakhand in 2013 or J&K a few months ago. Today, every IAF command in the country has a PTU each. According to Sandhu, the Eastern Air Command is developing four more as it makes sense to have them wherever airlift facilities exist.
The original PTU has undergone several systematic upgrades with efforts to reduce its weight by using reinforced tubular, aluminum foldable-frames for the stretcher-trolley, replacing the considerably heavy Nickel-Cadmium inverter batteries with compact self-sustaining ones, fitment of light-weight, long-sustaining oxygen cylinders and other self-sustaining monitoring units including defibrillator, suction machines and infusion pumps among other contraptions.
According to Joshi, critical care is a dynamic specialty as an aircraft can't be treated as an 'ambulance with wings'. He explains the 'G' forces encountered that could increase the inter-cranial pressure of a patient along with the varying cabin pressure and temperatures. Such has been the confidence of doctors that even a two-day old infant of an air force personnel was successfully airlifted using the PTU from Shillong to Army Research and Referral Hospital, New Delhi recently.
