Considering the number of nuclear weapons that have been created since 1945, it is something of a miracle that up to now there hasn't been a major nuclear explosion involving them, so how safe are nuclear weapons?

Technically---they are extremely extremely safe---.

They are controlled by an extremely sophisticated triggering mechanism that follows a sequence which has to be perfect 100 times out of 100---.

one saving grace is that the west where they went the extra mile to make sure safety was paramount, I wonder how it was done in the Soviet Union? where system worked differently.
